Will Testosterone Therapy Lower My Sperm Count? Donna Lee and Dustin Fontenot, PA-C Answer a Listener Question

In this segment,  Donna Lee and NAU Urology Specialists' Dustin Fontenot, PA-C answer a listener's question about fertility and testosterone treatment. While many assume that testosterone therapy would increase sperm production, the opposite is usually the case--the body's feedback mechanisms tend to inhibit sperm production in response to surges in testosterone. What does that means for men in their child-bearing years? Anyone beginning testosterone therapy needs baseline labs, and semen analysis should be part of that baseline if you ever plan on conceiving. There is a relationship between low testoerone (which leads people to seek hormone treatment) and low sperm count already, so taking testosterone could reduce an already low sperm count to almost zero. If you are actively seeking to get pregnant, there are ways to boost your natural sperm and testosterone production with adjunctive medications. There are also medications like Clomiphene which boost testosterone in fertility-sparing ways. While stopping testosterone treatment is often enough to bring sperm counts back up, there is always the back-up option to freeze sperm before beginning treatment. Importantly, whether you're hoping to conceive tomorrow or ten years from now, the best thing you can do is talk to your doctor about how your hormone treatment might affect your fertility and what plans you can make to preserve it.

Dr. Mistry and Shawn Elander, FNP of Victory Medical on the Remarkable Advances in HIV/AIDS Treatment and Prevention

Today, Dr. Mistry and Shawn discuss the history of the HIV/AIDS epidemic in the US. Interestingly, the great improvements in both prevention and treatment of recent years may have caused HIV infection rates to increase once again--partially due to more relaxed attitudes toward HIV as it's stigma wanes. While there is no substitute for sexual education and safe sex practices, two daily preventative medications, Truvada and Discovy, are available to those at higher risk of contracting HIV through sex and both are remarkably effective. There are also exposure medications that are more than 99% effective at preventing infection when taken within 72 hours of exposure.  For those living with HIV/AIDS, there have been amazing advances in those drug therapies as well. It is now possible for many patients to keep their HIV levels undetectable with proper treatment. Kidney Stoned: Dr. Mistry Explains Why Tiny Stones Hurt So Much, And What You Can Do For The Pain

In this segment,  Dr. Mistry and Donna Lee answer a listener's question about treatment for kidney stones. As many people are painfully aware, passing a kidney stone can be a nasty business. The pain from kidney stones can be felt in the flank, the bladder, the testicle, or even in the abdomen. Typically, this pain is caused when the stone moves through the narrow tube from the kidney down to the bladder, NOT when it passes through the urethra during excretion. Furthermore, the size of the stone does not necessarily correlate with the pain it causes--even 1-2 millimeter stones can be very painful while some people pass much larger ones without difficulty. Anti-inflammatory medications like Toradol or ibuprofen can help patients cope with this pain. If you haven't passed your stone within two weeks, it is unlikely to pass on its own and will likely require surgical intervention.  Remember that your pain may abate BEFORE your stone has been passed--don't assume it is gone unless you've actually seen it! When in doubt, a follow-up X-ray or test for blood in the urine is always the safest way to ensure your kidney stone is gone for good.

D*mned If You DO or You DON'T: Dr. Mistry Answers a Listener's Questions About STD's and "Prostate Congestion" (aka "Blue Balls")

In this segment,  Dr. Mistry and Donna Lee answer a listener's question about difficulty ejaculating and severe pelvic and abdominal pain--symptoms he developed after going several years without sexual intercourse. Dr. Mistry explains that this condition, often called "Blue Balls" when experienced by younger men, could also be described as "prostate congestion." When a man doesn't ejaculate for extended periods of time, his prostate can become "backed up" with ejaculate and this can lead to symptoms of sexual dysfunction and pain. It can also be exacerbated by musculoskeletal issues. 

Dr. Mistry also discusses the most common sexually transmitted diseases, or STD's, that he treats in his practice. While most STD's can be handled by urgent or primary care providers, Dr. Mistry and his team at NAU Urology Specialists offer compassionate care and/or a second opinion to anyone seeking speciality treatment. Too Young for ED? What To Do When Erectile and Sexual Dysfunction Affect Young Men

In this segment,  Dr. Mistry and Donna Lee discuss erectile dysfunction in young men. We all know ED is an issue many men face as they age. But what happens when you have symptoms of ED or sexual dysfunction in the "prime" of your sexual years? Dr. Mistry explains common complaints of young men, many of whom discover problems when they first begin to masturbate and/or have sex. Some of these issues are surprisingly simple to address, like frenulums that are too tight. Others may seem like physiological issues, but may in fact stem from psychological or sociocultural factors that can be treated with education and/or sex therapy.
